Please consider completing the short questionnaire that follows, it should take approximately five minutes to complete. The information collected will be used for program improvement purposes. Participation is voluntary. Should you decide to participate, your anonymity and the confidentiality of your responses will be maintained at all times. Only the instructors will review the information collected. Once the information is reviewed, it will be discarded electronically.
